You can download this file then complete the fields on your computer and email it to admin@studentguard.com.Or, you can print it out, complete the fields by hand and fax the completed form to 1-866-329-8447 or (905) 731-6676.

A Confirmation Listing will be sent to your email address or fax number as noted on your Enrollment Form within 24 hours.

Invoices will be issued to your school or organization. Payment terms are net 30 days.

ID Cards and Policy packages will be sent to the address you provide on the Enrollment Form within 48 hours.
