An Extension is a request for a continuation of coverage. Coverage
must be continuous in order to be considered an 'Extension' which
means you can have no breaks in coverage. If there are breaks in
coverage, your application will be considered a new application
Please send us your request for an Extension of coverage
in writing at least one week before the End Date of your existing coverage.
Include your previous policy number and complete the application in full so that we are able
to process your request
Note: all Extension requests are subject to approval by the Plan Administrator and/or insurer.
Once your Application for Extension and payment have been processed we will notify you by fax
or email, then mail your ID Card and Policy package to the address you provide on the application form.
